Can Botox Help Depression? What the Research Really Says

Most people think of botulinum toxin anti-wrinkle injections as something that softens frown lines and reduces the appearance of wrinkles.

But over the last two decades, researchers have been investigating a very different question.


Could treating the muscles responsible for frowning actually improve symptoms of depression?

It sounds surprising, but this isn't an internet myth or simply another social media trend. It is a genuine area of medical research, with multiple randomised controlled trials, systematic reviews and meta-analyses now published (Schulze et al., 2021; Arnone et al., 2021).


So, is there actually anything in it?

Possibly, yes.

But, like many eye-catching medical headlines, the reality is a little more nuanced.


Where did this idea come from?

The theory is known as the facial feedback hypothesis.

In simple terms, it suggests that our facial expressions don't just reflect how we feel. They may also influence how we feel.


Think about it this way.

When we're upset, stressed or frustrated, we naturally contract the muscles between our eyebrows, particularly the corrugator and procerus muscles that create our frown lines.

Researchers wondered whether temporarily relaxing those muscles with botulinum toxin might reduce the negative emotional feedback being sent back to the brain.

It sounds almost too simple.

Yet psychologists have explored versions of this idea for well over a century, and modern neuroscience has provided several possible biological explanations. Some brain imaging studies have even suggested changes in activity within emotional processing centres such as the amygdala after treatment, although exactly how this happens is still being investigated (Li et al., 2021).


What does the research actually show?

Several randomised controlled trials have compared botulinum toxin injections into the glabellar (frown) muscles with placebo injections in people diagnosed with major depressive disorder.

The results have generally been encouraging.

One pooled analysis combined data from three clinical trials involving 134 patients. Six weeks after treatment, researchers found:

  • a 54% response rate in the botulinum toxin group compared with 11% in the placebo group

  • approximately 30% of patients achieved remission, compared with around 7% receiving placebo (Magid et al., 2015).

Those figures certainly catch your attention.

But they're only part of the story.


The evidence is promising... but it isn't definitive

One of the biggest challenges is that the studies have generally been relatively small.

Different trials have used different doses, different patient populations and slightly different methods, making direct comparisons difficult.

Since those earlier studies, researchers have combined the available evidence in several systematic reviews and meta-analyses.

A 2021 review published in the Journal of Psychiatric Research concluded that botulinum toxin produced greater improvements in depressive symptoms than placebo across five randomised controlled trials. However, the authors also highlighted significant variation between studies and concluded that further research is needed before the treatment's role can be fully understood (Schulze et al., 2021).

A second meta-analysis reached similar conclusions, finding statistically significant improvements in depressive symptoms while also reporting that treatment was generally well tolerated. Again, the authors stressed that larger, higher-quality clinical trials remain necessary (Li et al., 2021).


In other words...

The research is genuinely encouraging.

But it isn't yet definitive.


Why might Botox affect mood?

Several theories have been proposed.

The most widely accepted remains the facial feedback hypothesis.

If the muscles responsible for expressing sadness, anger or distress are temporarily relaxed, the brain may receive fewer negative emotional signals.

Other researchers have suggested changes in emotional processing within the brain itself.

At the moment, these remain theories, not established facts.

Researchers are still trying to understand exactly why some patients appear to benefit more than others.


So... does Botox treat depression?

This is probably the most important question.

The honest answer is: Not at present.

Botulinum toxin is not licensed in the UK as a treatment for depression or anxiety, and it should never replace evidence-based mental health care such as assessment by a GP, psychological therapies or prescribed medication where appropriate. NICE continues to recommend established psychological and pharmacological treatments depending on the severity of depression (NICE, 2022).


What the current evidence suggests is that Botox may have antidepressant effects in some patients.

What it does not tell us is that botulinum toxin is now an established treatment for depression.

Those are two very different statements.


What about cosmetic patients?

This is where things become particularly interesting.

Many cosmetic patients genuinely report feeling:

  • more confident

  • happier with their appearance

  • more refreshed

  • more comfortable in social situations

Those experiences are real.

Feeling better about how you look can absolutely improve confidence and wellbeing.

But that's different from saying botulinum toxin directly treats depression.

Researchers are still trying to answer that much bigger question.


The real takeaway

This is one of the most fascinating areas of botulinum toxin research.

Multiple clinical trials have reported improvements in depressive symptoms.

Several systematic reviews and meta-analyses suggest those findings deserve to be taken seriously.

At the same time, the research remains relatively small, larger studies are still needed, and botulinum toxin is not currently an approved treatment for depression.


So the sensible position right now is:

  • scientifically fascinating

  • supported by promising evidence

  • worthy of further research

  • not yet established clinical practice

Sometimes the most honest answer isn't "yes" or "no."

It's simply:

"The evidence is encouraging, but we're not there yet."
